چکیده

Objective: To determine the role of and indications for peroral cholangioscopy using SpyGlass system in differential diagnosis biliary tract lesions case strictures, based on literature data analysis our own experience. Materials methods: Peroral is mainly used lesions. During cholangioscopy, we carefully consider gross signs damage to bile duct mucosa: abnormal capillary vascular pattern, granulation tissue other types proliferation, palpatory characteristics wall. Morphology should also be verified during image-guided intraductal forceps biopsy. Results: endoscopy significantly increases effectiveness between various strictures. Direct examination mucosa with optical biopsy morphological verification sensitivity specificity stricture type determination up 83.3%–96% 90.9%–99%, respectively. The diagnostic value malignant benign exceeds endoscopic retrograde cholangiopancreatography, even fluoroscopy-guided ducts. Conclusions: its enormous potential plays an important management patients diseases bilio-pancreatoduodenal area, including We formulated key experience this technique pathology, nondifferentiated

Recent developments in choledochoscopy: technical and clinical advances

Peroral cholangioscopy has become an important tool in the diagnosis and treatment of a variety of biliary diseases, ranging from indeterminate biliary strictures to bile duct stones. Although the first cholangioscopy was performed in the 1970s, recent technological advances have provided us with cholangioscopes that yield high-resolution images, possess single-operator capability, and have ult...

Peroral cholangioscopy: use of a Roth Net retriever for difficult biliary stones.

Bile duct stones are routinely removed during endoscopic retrograde cholangiopancreatography (ERCP) with biliary sphincterotomy, using standard balloon or basket extraction techniques. However, in approximately 10%–15% of patients, bile duct stonesmay be difficult to remove owing to challenging access to the bile duct, large bile duct stones (>15mm in diameter), intrahepatic stones, or impacted...

The efficacy of peroral cholangioscopy for difficult bile duct stones and indeterminate strictures: a systematic review and meta-analysis

BACKGROUND AND STUDY AIMS Current evidence supporting the efficacy of peroral cholangioscopy (POC) in the evaluation and management of difficult bile duct stones and indeterminate strictures is limited. The aims of this systematic review and meta-analysis were to assess the following: the efficacy of POC for the therapy of difficult bile duct stones, the diagnostic accuracy of POC for the evalu...

Surgical treatment of cicatricial biliary strictures.

BACKGROUND/AIMS Cicatricial biliary strictures are usually associated with high morbidity and mortality rates, frequently related to technical difficulties of their surgical repair, mainly in hilar lesions. Interference with bile duct blood supply during surgical attempts for correction is a major factor for unsuccessful results. The aim of this study is to evaluate, after an extended follow-up...
